Innovative Programs Supported by Internal Grants

Princeton HealthCare System (PHCS) Foundation's Internal Grants Program is a competitive grant initiative that funds programs, services, and equipment at Princeton HealthCare System that enhance or improve clinical outcomes­ or patient satisfaction. In 2014, PHCS received a wide array of exceptional proposals, making the selection process especially competitive and challenging for the Internal Grants Review Committee. The PHCS Foundation received 39 proposals requesting more than $811,000 and granted 12 awards totaling $250,000.

"This program continues to represent a funding opportunity for philanthropists and external funding sources who may wish to help us to expand our capacity to implement impactful improvements in patient care and service of our dedicated staff."

PHCS Foundation's Internal Grants recipients include the following:

Princeton HomeCare Services' "Best Practices in Home Health Care" Training:  A two-day Continuing Education Unit (CEU) workshop, designed to provide the PHCS HomeCare team with the latest evidence-based techniques. 

Community Education & Outreach's "Breast-Feeding Education and Lactation Support Program": A program providing mothers who deliver at University Medical Center of Princeton (UMCP) with support, education, and materials that promote exclusive breast-feeding of their babies. 

UMCP Behavioral Health Work Group's "Crisis Intervention and Prevention Training" (involving the PHCS Nursing Department and PHCS Department of Psychiatry):  A four-day, nonviolent crisis intervention instructor certification program to create a core group of instructors to offer ongoing training to PHCS staff and physicians. 

Performance Improvement, Infection Control, and Patient Safety's "Compassion in Action: Schwartz Center for Compassionate Healthcare Rounds":  An initiative to develop an interdisciplinary Schwartz Center Rounds ®  as a forum for the healthcare team to discuss difficult emotional and social issues. Schwartz Center Rounds® are now taking place in more than 350 healthcare facilities throughout the United States and in more than 60 institutions throughout the United Kingdom-offering healthcare providers the opportunity to improve connections with patients and colleagues through addressing the human dimension of medicine. 

Edward & Marie Matthews Center for Cancer Care's "Comprehensive Survivorship Program":  A comprehensive program to support post-treatment cancer survivors to successfully manage/navigate their health following completion of treatment. 

Critical Care Unit and Inpatient Rehab Services' "Early Mobilization in the Critical Care Unit at UMCP" Program:  A multidimensional early mobilization initiative to meet evidence-based practice guidelines to decrease Intensive Care Unit acquired illness and related morbidity/mortality. 

Center for Eating Disorders Care's "Enhancing Evidence-Based Treatment for Eating Disorders" Program:  An initiative to adapt family-based treatment (FBT) and cognitive remediation therapy (CRT), both evidence-based models, for UMCP's Center for Eating Disorders Care. 

Laboratory's "High-Efficiency Phlebotomy Carts":  New phlebotomy carts for each UMCP hospital floor to allow phlebotomists to follow best practice by operating at the bedside. 

HomeCare/Rehab Inpatient Services/Outpatient Rehab Network's "Joint Replacement Rehabilitation" Program:  An ongoing education program for those who care for joint replacement patients throughout the patients' course of treatment. 

Bristol-Myers Squibb Community Health Center's "Prescription Assistance Program 2014":  Prescription expense relief for uninsured and underinsured patients of the Bristol-Myers Squibb Community Health Center. 

Telemetry's "Project Re-engineered Discharge (RED)":  The expansion of the successful Project RED pilot, which has resulted in decreased readmission rates and improved patient satisfaction scores. 

Pharmacy's "UP-TEAM: Utilizing a Pharmacy-Technician to Achieve Optimal Medication" Program:  A robust medication reconciliation process to enhance patient care, safety, and satisfaction as patients are discharged home. Medication reconciliation involves finding out what medications a patient is taking now as compared with those received in the hospital. This process represents a National Patient Safety Goal.
