Hospital Transition Planning and Implementation

Transition planning is the process of assessing, planning, organizing, managing and prioritizing the thousands of tasks that must be completed to ensure a safe and successful move into a new hospital or expanded facility. PHI can provide project management services and tools to help clients execute a safe and orderly transition. PHI can train and motivate your team to successfully accomplish every step of your hospital transition.

Examples of services include:

  • Working with your staff to interview hospital leadership and department representatives as they develop a comprehensive list of activities to be completed prior to opening the new hospital
  • Structuring committees, preparing and coordinating the required committee meeting materials and providing on-site and remote support
  • Helping to determine which critical tasks need additional attention, resources, and support
  • Developing and implementing a “dress rehearsal” for all staff that will be involved in the move and in ongoing operations. The dress rehearsal provides an opportunity to test scenarios with mock patients and to evaluate new workflows, operation plans, systems integration, staff training effectiveness, and equipment functionality in the new facility
  • Reviewing project documents, design information, project priorities, occupancy phasing, and equipment planning materials to ensure successful transitioning
  • Organizing and supporting the transition team to develop training materials
  • Working with project leadership to develop a regulatory plan and implementation schedule to accomplish the required deliverables for government and other accreditation entities, such as Joint Commission International®*

Ongoing and On-Call Consulting Services

PHI offers ongoing training and consulting for your project’s evolving needs. Consulting can include an on-call team available 24 hours a day, 7 days a week to help you resolve problems as they arise. Your on-call staff can include physicians, nurses, educators, human resource professionals, facility managers, and safety personnel.

  • On-site training and consulting services
  • Training at University Medical Center of Princeton's healthcare campus
  • 24/7 on-call team available to respond via phone, email, and Skype to your immediate needs
  • Regular on-site teleconferencing and web meetings to assess project progress and address problems
  • Distance education and training via teleconference and web meetings for physicians, nurses, administrators, and healthcare facility planners
