Tips for Adults Attending an Evaluation Appointment:

Please bring two forms of identification, your insurance card, driver’s license, and the names, addresses, and phone numbers of your medical doctor, pharmacy, therapist, psychiatrist, and any other providers we may need to coordinate with regarding your care, e.g. probation officer.

Please bring a list of all medications and dosages. This evaluation will take approximately 2-1/2 hrs. Please plan to start our group program the day after your evaluation for three or five days per week based on the doctor’s recommendation.

Tips for Children and Adolescents Attending an Evaluation Appointment:

Please bring two forms of identification for the parent/legal guardian who is bringing your child. You will also need your child’s insurance card and the names, addresses, and phone numbers of your child’s pediatrician, pharmacy, therapists, psychiatrists, school, guidance counselor, and any other providers we may need to coordinate with regarding your care, e.g. probation officer.

Please bring a list of all your child’s medications and dosages. This evaluation will take approximately 3-1/2 hours. Please plan for your child to start our group program the day after his/her evaluation for three to five days per week based on the doctor’s recommendation.

If You Need to Cancel or Reschedule an Evaluation Appointment:

Please call Admissions at 888.437.1610 if you need to cancel or reschedule an evaluation appointment.

We require 24 hr. notice for cancellations or requests for rescheduling of appointments. Without 24 hr. notice, a new evaluation time may not be available for more than 30 days.
