HealthCare Partners, LLC (PHCP) is a non-profit limited liability
corporation whose sole equity owner is Princeton HealthCare System Holding
Company, Inc. Princeton HealthCare Partners LLC's purpose is to serve as an
Accountable Care Organization (ACO). PHCP has as its Members, Princeton
HealthCare System, a subsidiary of Princeton HealthCare System Holding, Inc.,
Princeton HealthCare Affiliated Physicians, PC (d.b.a., Princeton Medicine),
and physician practices in the community served by PHCP.

the organization are to enhance the quality of health care and to reduce the
historically steady increase of health care costs by acting as a facilitator
and provider of the foundation needed to assist its participating members to
transition from fee for service model to a model focused on population health.

PHCP's care coordination program will engage patients, families and caregivers
in facilitating beneficiary movement through the continuum of care. The process
establishes proper patient length of stay and reduces readmissions while
ensuring the safe progression of care. This approach provides a
patient-centered care delivery model that will be well-coordinated, accessible
treatment, alternative options, and an emphasis on self-management skills for
all beneficiaries. The primary care practices remain the center of care
coordination for all assigned patients. Transition coordination of patients to
an acute setting, or from an acute setting to a medical home in the primary
care setting, is accomplished through use of Princeton HealthConnect®.
