A Team Approach to
Your Care

The Diabetes Management Program provides expert care to adults and children of any age to help you learn how to live with and manage diabetes. The program is guided by a board certified endocrinologist and staffed by a multidisciplinary team of diabetes specialists who work closely with you, your physician and other healthcare providers. Our team of Certified Diabetes Educators includes registered nurses and registered dietitians.

  • Diabetes care and management for pre-pregnancy and during pregnancy
  • Nutrition guidance and comprehensive meal planning

  • Professionally facilitated monthly support groups

Helping You Gain Control Over Diabetes

Modern research has provided an unprecedented range of effective treatments for diabetes, greatly reducing the risks of serious complications.

Finding a healthy personal balance -- learning to control your blood sugar levels through a combination of good nutrition, exercise and often medication -- is key to successfully managing your diabetes.

Our customized outpatient self-management program includes an initial personal assessment. Working closely with your referring healthcare provider, our caring staff will educate you about diabetes, home glucose monitoring, healthy nutrition, appropriate exercise and careful medication management, if needed, so that you can effectively manage your diabetes and enjoy an improved quality of life. Additional programs on special topics are offered periodically, such as healthy eating, heart health and stress management.

Continuum of Care

When patients are hospitalized at University Medical Center at Princeton, either with existing or new onset diabetes, the Certified Diabetes Educators of the Diabetes Management Program are available for consultation. They will collaborate with your admitting physician and inpatient healthcare team, to ensure the optimum level of diabetes care, treatment and education during your stay. Upon discharge, your healthcare provider can then refer you to the comprehensive outpatient Self-Management Education Program for additional instruction to assist you in achieving optimal diabetes outcomes.

Our enhanced program also includes Medical Nutrition Therapy (MNT). This service is distinct and separate from the nutrition education included in the comprehensive management course. It is intended to complement our traditional program and address personal nutrition needs.

Follow-Up Care

We keep in contact with you after your initial services to inquire about your progress and/or ongoing needs. We encourage annual follow-up visits to our program.

Referrals & Insurance

Participation in the outpatient Diabetes Management Program requires a referral from your physician/healthcare provider. Medicare, Medicaid and most third-party insurances are accepted.

Nationally Recognized Care

The Diabetes Management Program at University Medical Center at Princeton is recognized by the American Diabetes Association as a quality diabetes self-management education program. This award is based on a rigorous clinical review process every three years to ensure high standards of diabetes care and education.
