A Team Approach to Your Care

Under the guidance of our Medical Director – a board certified endocrinologist – a multidisciplinary team of diabetes specialists works with you and your physician to help you successfully manage your diabetes. Our team — all of whom are certified diabetes educators — includes registered nurses, registered dietitians, exercise specialists and other healthcare professionals. The entire team works together to bring you a comprehensive approach to diabetes management with a high level of care and personal concern.

Helping You Gain Control Over Diabetes

Diabetes is the sixth leading cause of death by disease in the United States. It is a serious health problem that, left untreated, can lead to heart disease, kidney failure, amputation, blindness, impotence and a host of other serious complications. Finding a healthy personal balance – learning to control your blood sugar levels through a combination of good nutrition, exercise and sometimes medication – is key to successfully managing diabetes.WomanOnBike

Our customized outpatient self-management program includes an initial one-on-one diabetes assessment and provides you with an overview of the disease. Working closely with your personal or referring physician, our staff will help you learn to monitor your blood sugar levels and achieve a healthy balance of good nutrition, appropriate exercise and careful medication management, so that you can effectively take charge of your diabetes and enjoy a higher quality of life. Special programs and classes can help you address individual concerns of everyday living. We also offer risk assessments, eye and foot screenings, and a host of additional programs and services.

As a patient in the Diabetes Management Program, you will benefit from a continuum of services available through University Medical Center at Princeton and Princeton HealthCare System.

If you are ever hospitalized at University Medical Center at Princeton, the Diabetes Management Program team will work in collaboration with your admitting physician to ensure the highest level of diabetes care and treatment during your stay. As an inpatient, your diabetes history and current treatment protocol will be accessed promptly to ensure you receive optimal diabetes care at all times.

Referrals & Insurance

Participation in the outpatient Diabetes Management Program requires a referral from a physician. Medicaid, Medicare and most third-party insurances are accepted.

Nationally Recognized Care

The Diabetes Management Program at University Medical Center at Princeton is recognized by the American Diabetes Association as a quality diabetes self-management education program. This award is granted for a three-year period and is based on a rigorous clinical review process to assure high national standards are met.
